Are you passionate about supporting children and youth with disabilities and helping families navigate complex systems and resources? The Support and Service Coordinator (SSC) role offers an opportunity to build meaningful relationships with youth and families while helping them access the services, supports, and resources they need to thrive in their homes and communities.
As an SSC, you will serve as a trusted partner and advocate for children, youth, and their families enrolled in Wisconsin’s Children’s Long-Term Support (CLTS) Waiver Program. You’ll help families identify their strengths, goals, and needs, connect them with resources, and coordinate individualized services that promote independence, inclusion, safety, and quality of life.
This is a relationship-driven role for someone who is compassionate, organized, resourceful, culturally responsive, and comfortable advocating for others.
We’re looking for someone who brings both compassion and strong organizational skills to their work. Successful SSCs are comfortable building relationships with diverse families, navigating complex systems, solving problems, and advocating for children and youth.
You’ll bring:
The SSC reports directly to the CLTS Supervisor and works collaboratively with a broad network of partners. This includes youth and families, formal and informal supports, service providers, funders, county staff, schools and Milwaukee Public Schools (MPS), disability-serving organizations, community agencies, and other professionals involved in supporting the youth.
The role includes participation in team meetings, staffing, consultations, required training, and ongoing professional development. SSCs may also provide coverage and support for colleagues and their families when needed.

As a Support and Service Coordinator, you’ll help turn those goals into meaningful opportunities by connecting families with the right resources, advocating for their choices, and helping youth participate fully in their homes and communities.
