Home Health Registered Nurse (RN)
One Home Health —Elgin, IL

One Home Health is looking for an experienced Registered Nurse to join our home health team. This isn't an entry-level role. We need someone who already knows how to manage a caseload, think on their feet in a patient's home, and work independently without a nurse's station down the hall.

What you'll do

• Manage a caseload of home health patients, completing skilled nursing visits, assessments, and care plan updates in the field
• Perform admissions, recertifications, and discharges, with accurate and timely OASIS documentation
• Coordinate with physicians, therapists, and the interdisciplinary team on changes in patient condition
• Educate patients and family caregivers on disease management, medications, and home safety
• Respond to changes in patient status and escalate appropriately
• Maintain documentation that meets Medicare, state, and agency compliance standards

What you need

• Active, unencumbered RN license in IL
• Minimum of [1-2] years of RN experience, with home health experience required
• Working knowledge of OASIS documentation and Medicare home health guidelines
• Comfortable working independently in patients' homes across [service area]
• Reliable transportation and valid driver's license
• Strong clinical judgment and the ability to prioritize a full caseload without close supervision

Preferred

• Experience with Axes
• Certification in wound care, oncology, or chronic disease management
• Bilingual candidates encouraged to apply
